O'Neal, 46, is charged with six felony and misdemeanor counts including driving under the influence causing injury, possession of a firearm by a felon and possession of a controlled substance.
Prosecutors allege O'Neal was under the influence of drugs Aug. 2, when he veered into oncoming traffic and collided with another vehicle.
The other motorist suffered head and back injuries.
Toxicology testing revealed that O'Neal had amphetamines, cocaine, marijuana and Xanax in his blood.
At the time of the crash, he was on probation for a previous DUI, the prosecutor said.
Rock cocaine and loaded weapons were found in the car, according to police.
O'Neal has appeared in several films, including "April Fool's Day," "Assault of the Killer Bimbos" and "Ghoulies III: Ghoulies Go to College."
In the past, he has publicly discussed his family's tumultuous life, including his father's difficult marriage with his stepmother, the late icon Farrah Fawcett.
