The Florida Department of Health in Lake County will offer free required immunizations for students in August.
All children attending public or private school must have proof of immunizations — a Florida Certificate of Immunization (Form 680) — before they are allowed to attend school. The form is supplied by family physicians or local county health departments.
The following immunizations are required: diphtheria, pertussis, tetanus, polio, measles, mumps, rubella, hepatitis B and varicella (chickenpox).
Children must be accompanied by a parent or guardian and must have their shot records.
Children need a fourth polio vaccination after they turn 4 and before they begin kindergarten, which went into effect in 2011.
Children entering, attending or transferring to the seventh, eighth, ninth, 10th, 11th or 12th grade are required to have a Tdap vaccine — a dose of tetanus-diphtheria-pertussis.
Immunizations will be offered at the following locations:
•Umatilla health center, 249 E. Collins St. from 8:30 a.m. to 12:30 p.m. Aug 9 and 16. Details: 352-771-5500.
•Leesburg health center, 2113 W. Griffin Road from 9 a.m. to 1 p.m. Aug. 9. Details: 352-360-6548.
