"Sentara to Home," is a pilot program recently introduced at Sentara Norfolk General Hospital for cardiac patients.
The service delivers prescription medications and portable home medical supplies to patients' rooms prior to discharge. On request, the program's "transition specialists" will work with physicians to arrange follow-up office visits and for Sentara Home Care Services to deliver larger items.
As a complement to the program, the "Sentara to Home" retail store has opened near the hospital's lobby. It offers outpatient prescriptions and over-the-counter medications, plus a broad variety of home care supplies. These include a pouring device for gallon milk jugs, comfort wraps that can be heated in the microwave, and motorized up-and-down easy chairs.
If successful, the program will expand to other Sentara hospitals.
