Application for Licensure denied. The Board determined that there are sufficient grounds for denial of applicant’s application for licensure. Date of Action: June 23, 2011

Prafull K. Dave, M.D.
Area of Practice: Neurology (Frederick, MD)
Reprimand; Probation with terms and conditions. The Board found that the physician failed to meet the standards of quality medical care and failed to keep adequate medical records in six patients reviewed. Date of Action: June 23, 2011

John R. Durocher, M.D.
Area of Practice: Internal Medicine (Rock Hall, MD)
Suspension for 3 months, then probation for a minimum of two years with a permanent prescribing restriction and additional terms and conditions. The Board found that the physician failed to meet the standards of quality medical care and failed to keep adequate medical records in eleven pain management patients reviewed. Date of Action: June 23, 2011

Maria C. Gaviria, M.D.

Termination of probationary terms and conditions imposed by the Consent Order, dated
February 26, 2009. The physician has successfully complied with the terms and conditions

James L. Hooper, M.D.
Area of Practice: Physical Medicine & Rehabilitation (Owings Mills, MD)
Summary Suspension is terminated. Suspension for one year from the date of the order; upon termination of the suspension, Probation for a minimum of 2 years with conditions including limitations on prescribing CDS, supervision for a minimum of one year and a chart and/or peer review. The Board found that the physician prescribed drugs for illegitimate medical purposes, is guilty of unprofessional conduct in the practice of medicine and that he is professionally incompetent to practice pain medicine. The physician repeatedly provided his patients with prescriptions for large amounts of Controlled Dangerous Substances, failed to perform or document any physical examination, and prescribed CDS without sufficient medical

Application for Licensure denied. The Board determined that there are sufficient grounds for

Area of Practice: Family Practice & Geriatric Medicine (Baltimore, MD)
Suspension for 1 year, with an active suspension for a minimum of 6 months. Fine of $10,000. After active suspension, Probation for a minimum of 2 years with an on-going prescribing restriction and additional terms and conditions. The Board found that the physician failed to meet the standards of quality medical care in three patients reviewed, failed to keep adequate medical records in two patients reviewed and violated a prior Consent Order by prescribing chronic pain medication in three patients reviewed. Date of Action: June 22, 2011

Robert J. Reynolds, M.D.
Area of Practice: Internal Medicine (Candler, NC)
Suspension, which was immediately stayed, is terminated. On December 31, 2008, based upon a disciplinary action by the North Carolina Medical Board, the physician entered into a Consent Order under which his medical license was suspended, but the suspension was immediately stayed. Date of Action: June 2, 2011

Revocation; Summary Suspension issued on May 21, 2010 is terminated. The Board found the physician guilty of immoral conduct and unprofessional conduct in the practice of medicine based upon sexual misconduct. Date of Action: June 23, 2011

Permanent Revocation and a fine of $50,000. The Board found that the physician prescribed drugs for illegal or illegitimate medical purposes, is guilty of unprofessional conduct in the practice of medicine and that he is professionally incompetent to practice pain medicine. The physician repeatedly provided his patients with prescriptions for large amounts of CDS that they requested, in the absence of any illness that would reasonably call for those prescriptions, and in the absence of an adequate medical examination. Gerard M. Toso, M.D., No License
